- Name of patient
- Name and quantity of medication
- Signature or stamp of physician

- Address of patient
- Age of patient
- Sex of patient
- Diagnosis

Additional information:

In order to avoid side effects due to interaction of different drugs or possible contraindications please give as well:

1- Known allergies to medications (e.g. penicillin, codeine, aspirin, etc....)
2- Drugs you are currently taking.
3- Diagnosis of other current diseases, illnesses.
4- Medical conditions
5- Kindly please confirm that you are informed and aware that you must immediately contact your healthcare provider in case of any problem related to the drugs sent to you.
